Intimation of change.—Every service provider to persons with disabilities shall intimate to the State Commissioner, in Form VIII and within twenty-one days of the change, any change in the particulars furnished in the application for registration, and the State Commissioner shall amend the certificate accordingly on payment of a fee of one hundred rupees.
