Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the service provider to persons with disabilities shall apply to the State Commissioner in Form VIII for a duplicate, accompanied by a fee of two hundred rupees, and the State Commissioner shall issue a duplicate certificate marked as such within ninety days of the application.
