Application for registration.—Every service provider to persons with disabilities carrying on the provision of services to persons with disabilities shall make an application for registration to the State Commissioner in Form I, within sixty days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every certificate held by him on the date of the application, and shall keep a copy of the application and of the statement for forty-five days from that date.
