Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the institution for persons with disabilities shall apply to the Government of Uttar Pradesh in Form VIII for a duplicate, accompanied by a fee of fifty rupees, and the Government of Uttar Pradesh shall issue a duplicate certificate marked as such within fifteen days of the application.
