Application for registration.—Every institution for persons with disabilities carrying on the running of an institution for persons with disabilities shall make an application for registration to the Government of Uttar Pradesh in Form II, within forty-five days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every roster held by him on the date of the application, and shall keep a copy of the application and of the statement for sixty days from that date.
