Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the sports body shall apply to the National Anti-Doping Appeal Panel in Form A for a duplicate, accompanied by a fee of two thousand five hundred rupees, and the National Anti-Doping Appeal Panel shall issue a duplicate certificate marked as such within thirty days of the application.
