Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the genetic counselling centre shall apply to the Director of Health Services in Form 29 for a duplicate, accompanied by a fee of two hundred and fifty rupees, and the Director of Health Services shall issue a duplicate certificate marked as such within fourteen days of the application.
