Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insurance agent shall apply to the Insurance Information Bureau in Form IRDA-4 for a duplicate, accompanied by a fee of one thousand rupees, and the Insurance Information Bureau shall issue a duplicate certificate marked as such within forty-five days of the application.
