Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the service provider shall apply to the Local Complaints Committee in Form IV for a duplicate, accompanied by a fee of two hundred rupees, and the Local Complaints Committee shall issue a duplicate certificate marked as such within three days of the application.
