Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the blood bank shall apply to the Government of Assam in Form 19 for a duplicate, accompanied by a fee of five thousand rupees, and the Government of Assam shall issue a duplicate certificate marked as such within seven days of the application.
