Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the protection of the interests of debenture holders is carried on, the particulars of every balance sheet held by the applicant and such other particulars as are specified in Form CHG-1, and shall be signed by the applicant or his authorised agent.
