Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the corporate agent shall apply to the Insurance Ombudsman in Form IRDA-6 for a duplicate, accompanied by a fee of five hundred rupees, and the Insurance Ombudsman shall issue a duplicate certificate marked as such within ninety days of the application.
