Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which investment in securities by a foreign portfolio investor is carried on, the particulars of every research report held by the applicant and such other particulars as are specified in Form A, and shall be signed by the applicant or his authorised agent.
