Intimation of change.—Every policy-holder shall intimate to the Government of Andhra Pradesh, in Form IRDA-5 and within one hundred and twenty days of the change, any change in the particulars furnished in the application for registration, and the Government of Andhra Pradesh shall amend the certificate accordingly on payment of a fee of one thousand rupees.
