Surrender of certificate.—Every domestic worker who ceases to carry on domestic work for wages shall, within twenty-one days of such cessation, surrender the certificate of registration to the Medical Benefit Council in person or by registered post, and shall inform it in Form 5 of the manner in which every identity card held by him has been disposed of.
