Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the operator of a vaccination centre shall apply to the District Registering Authority in Form 41 for a duplicate, accompanied by a fee of five thousand rupees, and the District Registering Authority shall issue a duplicate certificate marked as such within ninety days of the application.
