Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the operator of a hospital shall apply to the Relief Commissioner in Form G for a duplicate, accompanied by a fee of one thousand rupees, and the Relief Commissioner shall issue a duplicate certificate marked as such within fifteen days of the application.
