Intimation of change.—Every operator of a hospital shall intimate to the Relief Commissioner, in Form F and within fourteen days of the change, any change in the particulars furnished in the application for registration, and the Relief Commissioner shall amend the certificate accordingly on payment of a fee of one hundred rupees.
