Surrender of certificate.—Every seaman who ceases to carry on service on a ship shall, within fifteen days of such cessation, surrender the certificate of registration to the Port Authority in person or by registered post, and shall inform it in Form 5 of the manner in which every crew list held by him has been disposed of.
