Application for registration.—Every annuity service provider carrying on the provision of annuities shall make an application for registration to the Central Board in Form 5, within sixty days of the commencement of these rules, accompanied by a fee of fifty rupees and a statement of every account held by him on the date of the application, and shall keep a copy of the application and of the statement for fifteen days from that date.
