Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the exempted establishment shall apply to the Controlling Authority in Form 11 for a duplicate, accompanied by a fee of two hundred rupees, and the Controlling Authority shall issue a duplicate certificate marked as such within ninety days of the application.
