Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the certifying medical authority shall apply to the Rehabilitation Council in Form IV for a duplicate, accompanied by a fee of five hundred rupees, and the Rehabilitation Council shall issue a duplicate certificate marked as such within thirty days of the application.
