Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the exempted establishment shall apply to the Employees' Provident Fund Appellate Tribunal in Form 10 for a duplicate, accompanied by a fee of two hundred rupees, and the Employees' Provident Fund Appellate Tribunal shall issue a duplicate certificate marked as such within sixty days of the application.
