Application for registration.—Every exempted establishment carrying on the maintenance of a private provident fund shall make an application for registration to the Employees' Provident Fund Appellate Tribunal in Form 5, within fifteen days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every permanent retirement account held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
