Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the institution for persons with disabilities shall apply to the State Commissioner in Form IV for a duplicate, accompanied by a fee of ten rupees, and the State Commissioner shall issue a duplicate certificate marked as such within thirty days of the application.
