Application for registration.—Every institution for persons with disabilities carrying on the running of an institution for persons with disabilities shall make an application for registration to the State Commissioner in Form IV, within fifteen days of the commencement of these rules, accompanied by a fee of ten rupees and a statement of every assistive device held by him on the date of the application, and shall keep a copy of the application and of the statement for one hundred and twenty days from that date.
