Surrender of certificate.—Every employee who ceases to carry on the receipt of wages in a covered establishment shall, within ten days of such cessation, surrender the certificate of registration to the Employees' Insurance Court in person or by registered post, and shall inform it in Form 5 of the manner in which every medical certificate held by him has been disposed of.
