Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the provision of services to persons with disabilities is carried on, the particulars of every register of beneficiaries held by the applicant and such other particulars as are specified in Form VII, and shall be signed by the applicant or his authorised agent.
