Application for registration.—Every service provider to persons with disabilities carrying on the provision of services to persons with disabilities shall make an application for registration to the Government of Bihar in Form VI, within thirty days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every certificate of disability held by him on the date of the application, and shall keep a copy of the application and of the statement for sixty days from that date.
