Surrender of certificate.—Every pharmacist who ceases to carry on the practice of pharmacy shall, within fifteen days of such cessation, surrender the certificate of registration to the District Registering Authority in person or by registered post, and shall inform it in Form 21 of the manner in which every prescription held by him has been disposed of.
