Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the third party administrator shall apply to the Insurance Ombudsman in Form IRDA-5 for a duplicate, accompanied by a fee of one thousand rupees, and the Insurance Ombudsman shall issue a duplicate certificate marked as such within forty-five days of the application.
