Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the operator of a vaccination centre shall apply to the Director of Health Services in Form 41 for a duplicate, accompanied by a fee of twenty-five thousand rupees, and the Director of Health Services shall issue a duplicate certificate marked as such within twenty-one days of the application.
