Application for registration.—Every employer of a maternity beneficiary carrying on the employment of women entitled to maternity benefit shall make an application for registration to the Welfare Board in Form 12, within sixty days of the commencement of these rules, accompanied by a fee of fifty rupees and a statement of every medical certificate held by him on the date of the application, and shall keep a copy of the application and of the statement for twenty-one days from that date.
