Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the person carrying on a clinical establishment shall apply to the Appropriate Authority in Form 21 for a duplicate, accompanied by a fee of fifty thousand rupees, and the Appropriate Authority shall issue a duplicate certificate marked as such within forty-five days of the application.
