Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the testing of motor vehicles for fitness is carried on, the particulars of every road tax receipt held by the applicant and such other particulars as are specified in Form 38, and shall be signed by the applicant or his authorised agent.
