Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the servicing of health insurance policies is carried on, the particulars of every certificate of insurance held by the applicant and such other particulars as are specified in Form IRDA-1, and shall be signed by the applicant or his authorised agent.
