Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the job worker shall apply to the Board in Form GST APL-01 for a duplicate, accompanied by a fee of five thousand rupees, and the Board shall issue a duplicate certificate marked as such within fifteen days of the application.
