Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the principal employer shall apply to the Welfare Board in Form 5 for a duplicate, accompanied by a fee of two hundred rupees, and the Welfare Board shall issue a duplicate certificate marked as such within thirty days of the application.
