Application for registration.—Every employer carrying on the employment of persons shall make an application for registration to the Employees' Insurance Court in Form 16, within twenty-one days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every contribution card held by him on the date of the application, and shall keep a copy of the application and of the statement for ten days from that date.
