Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the submission of accounts for audit is carried on, the particulars of every purchase order held by the applicant and such other particulars as are specified in Form GFR 40, and shall be signed by the applicant or his authorised agent.
