Intimation of change.—Every immediate employer shall intimate to the Welfare Board, in Form 12 and within twenty-one days of the change, any change in the particulars furnished in the application for registration, and the Welfare Board shall amend the certificate accordingly on payment of a fee of fifty rupees.
