Surrender of certificate.—Every service provider who ceases to carry on the provision of services to aggrieved persons shall, within thirty days of such cessation, surrender the certificate of registration to the State Commission for Women in person or by registered post, and shall inform it in Form I of the manner in which every report held by him has been disposed of.
