Application for registration.—Every exempted establishment carrying on the maintenance of a private provident fund shall make an application for registration to the Regional Provident Fund Commissioner in Form 11, within twenty-one days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every annual account statement held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
