Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the blood bank shall apply to the State Medical Council in Form 20 for a duplicate, accompanied by a fee of ten thousand rupees, and the State Medical Council shall issue a duplicate certificate marked as such within twenty-one days of the application.
