Surrender of certificate.—Every self-employed person who ceases to carry on the carrying on of a self-employed occupation shall, within ninety days of such cessation, surrender the certificate of registration to the Regional Board in person or by registered post, and shall inform it in Form 16 of the manner in which every contribution held by him has been disposed of.
