Application for registration.—Every certifying medical authority carrying on the certification of disability shall make an application for registration to the District Vigilance and Monitoring Committee in Form V, within twenty-one days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every register of beneficiaries held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
