Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the certifying medical authority shall apply to the District Vigilance and Monitoring Committee in Form III for a duplicate, accompanied by a fee of five hundred rupees, and the District Vigilance and Monitoring Committee shall issue a duplicate certificate marked as such within ninety days of the application.
