Intimation of change.—Every operator of a vaccination centre shall intimate to the Appropriate Authority, in Form 29 and within fourteen days of the change, any change in the particulars furnished in the application for registration, and the Appropriate Authority shall amend the certificate accordingly on payment of a fee of two hundred and fifty rupees.
