Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the employer shall apply to the Chief Inspector in Form V for a duplicate, accompanied by a fee of one hundred rupees, and the Chief Inspector shall issue a duplicate certificate marked as such within seven days of the application.
