Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the employee shall apply to the Employees' Insurance Court in Form 2 for a duplicate, accompanied by a fee of five hundred rupees, and the Employees' Insurance Court shall issue a duplicate certificate marked as such within thirty days of the application.
